REQUIREMENTS FOR REPORT OF DEATH:(Prepare in four sets)

FOR NATURAL CAUSE OF DEATH

1. Duly accomplished application for Report of Death and NOC form
2. English  and Arabic Death certificate
3. Original and copies of passport of deceased
4. English hospital Notification
5. If employed:  Certification on payments of end of service benefits
6. Cancellation of visa
7. Passport copy/id of informant

Note: If informant is not Next of Kin, authorization form  NOK is required

 FOR DEATH BY ACCIDENT OR NOT OF NATURAL CAUSE

1. Duly accomplished application for Report of Death and NOC form
2. English  and Arabic Death certificate
3. English forensic report /medico legal report and police report
4. Original and copies of passport of deceased
5. English hospital Notification(if available)
6. If employed:  Certification on payments of end of service benefits
7. Cancellation of visa
8. Passport copy/id of informant

Note: If informant is not Next of Kin, authorization from NOK is required

Fee: 200AED

Procedure:

1. Submit the requirements at the Civil Registry Unit (CRU) for evaluation of the consular officer. 
2. Pay necessary fees at the cashier and return receipt to the consular officer.
3. Wait for the issuance of the Report of Death and NOC.

Note: Despatch and Reference Numbers will be provided to the NOK as soon as available.  The Philippine Statistical Authority (PSA) requires these numbers when NOK requests copy of the Report of Death in PSA security paper.
