Married women who would like to use their married surname:

  1. Confirmed Online Appointment with QR code
  2. Personal appearance of applicant
  3. Original old passport
  4. Accomplished E-passport application form and OAV form
  5. Photocopy of :
    • Passport Data page (2 copies)
    • Visa page
  6. PSA-issued Marriage Contract (MC). Transcribed Marriage Contract from the LCR is required when entries in PSA Marriage Contract are blurred or unreadable.
  7. PSA-issued Report of Marriage if married abroad

IMPORTANT REMINDER:

    Pursuant to Department Order No. 37-03, all passports unclaimed by the applicants after six (6) months from the date of issuance shall be cancelled.

    CIVIL REGISTRATION

    Marriage 8:00am  3:30pm (Sundays to Wednesdays only)
    Death 8:00am  3:30pm
    Releasing of Civil Registration Documents

    For AAP, AUSF, ROM, LCCM

    4:00pm – 5:00pm (CRU Unit)

    For ROB

    (to be released with the new passport of the child)

    8:00am – 5:00pm (Passport Releasing Unit)

    For ROD

    ROD is released as soon as the applicant completes the requirements and pays the appropriate fees.
