Married women who would like to use their married surname:

- Confirmed Online Appointment
- Personal appearance of applicant
- Original old passport
- Printout online E-passport application form with barcode
- Photocopy of Passport Data page
- PSA-issued Marriage Contract (MC). Transcribed Marriage Contract from the LCR is required when entries in PSA Marriage Contract are blurred or unreadable.
- PSA-issued Report of Marriage if married abroad
- Additional supporting documents that may be required as necessary.

| Pursuant to Department Order No. 37-03, all passports unclaimed by the applicants after six (6) months from the date of issuance shall be cancelled. |
