Certificate of Appearance

(Confirmation that the applicant is presently residing/working in the UAE, and personally appeared before the official/s of the Consulate)

 

General Affidavit form
  1. Accomplished General Affidavit form stating the reason behind the need for Certificate of Appearance from the Consulate
  2. Two (2) copies of applicant's passport and residence visa
  3. Two (2) passport-size photo, if required by the agency/institution to which the Certificate will be submitted
  4. Any other document(s) that will be requested by the processor.

IMPORTANT REMINDER:

  • Please be reminded that applicant/s must personally sign the document/s in front of the officials of the Consulate General.
  • Any spurious or tampered documents submitted for Notarization/ Attestation are subject to outright seizure upon proper determination by the Consular Officer. Holder of said documents, as well as fixers will be prosecuted. Payment for the document found to be spurious will be forfeited.
