Affidavit of Consent to Travel

(Applicant's written statement confirmed by oath)

 
  1. Personal appearance of Filipino affiant/s (applicant/s).
  2. Completed Affidavit of Consent to Travel form and copy.
  3. Two (2) copies of affiant’s passport and residence visa.
  4. Any other document(s) that will be requested by the processor.

General instruction for applicants who wish to notarize affidavits and SPAs - "Applicants should prepare the text of their affidavits and SPAs prior to their transaction at the window. The processors will not draft and prepare them on behalf of the applicant."

IMPORTANT REMINDER:

  • Please be reminded that applicant/s must personally sign the document/s in front of the officials of the Consulate General.
  • Any spurious or tampered documents submitted for Notarization/ Attestation are subject to outright seizure upon proper determination by the Consular Officer. Holder of said documents, as well as fixers will be prosecuted. Payment for the document found to be spurious will be forfeited.
