(Statement of applicant on reason for registration of birth/marriage/death beyond prescribed period of 1 year)

**For civil registrations filed in the PH.

 

Affidavit of Delayed Registration of Birth/Marriage/Death
  1. Personal appearance of the affiant/s (applicant).
  2. Completed Affidavit of Delayed Registration form and copy.
  3. Two (2) copies of affiant's passport and residence visa.
  4. Any other document(s) that will be requested by the processor.

IMPORTANT REMINDER:

  • Please be reminded that applicant/s must personally sign the document/s in front of the officials of the Consulate General.
  • Any spurious or tampered documents submitted for Notarization/ Attestation are subject to outright seizure upon proper determination by the Consular Officer. Holder of said documents, as well as fixers will be prosecuted. Payment for the document found to be spurious will be forfeited.
