General Affidavit

(Applicant's written statement confirmed by oath)

 

General Affidavit form
  1. Personal appearance of affiant/s (applicant/s).
  2. Completed General Affidavit form and copy.
  3. Two (2) copies of affiant’s passport.
  4. Any other document(s) that will be requested by the processor.

General instruction for applicants who wish to notarize affidavits and SPAs - "Applicants should prepare the text of their affidavits and SPAs prior to their transaction at the window. The processors will not draft and prepare them on behalf of the applicant."

For example, Affidavit on Discrepancy of Name in passport and birth certificate.  Applicant must bring original and 2 photocopies of PSA birth certificate.

IMPORTANT REMINDER:

  • Please be reminded that applicant/s must personally sign the document/s in front of the officials of the Consulate General.
  • Any spurious or tampered documents submitted for Notarization/ Attestation are subject to outright seizure upon proper determination by the Consular Officer. Holder of said documents, as well as fixers will be prosecuted. Payment for the document found to be spurious will be forfeited.
