(Applicant's written statement confirmed by oath)

 

Affidavit of Denial Form
  1. Confirmed Appointment.
  2. Personal appearance of applicant.
  3. Completed Affidavit of Denial form and copy.
  4. Two (2) copies of applicant’s passport.
  5. Two (2) copies of NBI Form ICTD-F-003 regarding the pending criminal case or Court Order/decision/judgement/sentence/resolution/certification.
  6. Any other document(s) that will be requested by the processor.

IMPORTANT REMINDER:

  • Please be reminded that applicant/s must personally sign the document/s in front of the officials of the Consulate General.
  • Any spurious or tampered documents submitted for Notarization/ Attestation are subject to outright seizure upon proper determination by the Consular Officer. Holder of said documents, as well as fixers will be prosecuted. Payment for the document found to be spurious will be forfeited.
