Travel Document

For applicants with Expired /Damaged Passport (with urgent travel to the Philippines)

  1. Accomplished Travel Document Application Form
  2. Passport copy (expired/ damaged)
  3. Latest visa copy
  4. Flight ticket (direct flight to the Philippines)
  5. 4 copies of passport size photo

For applicants with lost passport (with urgent travel to the Philippines)

  1. Accomplished Travel Document Application Form
  2. Passport copy (lost passport)
  3. Latest visa copy
  4. Flight ticket (direct flight to the Philippines)
  5. 4 copies of passport size photo
  6. Affidavit of Loss ( can be accomplished in the Consulate)
  7. Police Report issued by Dubai Police with English translation (original and 2 photocopies)

Please take note that a travel document is valid for 30 days from issuance and can only be used one-way to the Philippines.

You can apply for the travel document in the Consulate without an appointment. The Consulate is open from Mondays- Thursdays 7:30 AM-3:30PM; Fridays 7:30 AM-12N.

IMPORTANT REMINDERS  

Pursuant to Department Order No. 37-03, all passports unclaimed by the applicants after six (6) months from the date of issuance shall be cancelled.

Renewal of Mutilated/ Damaged Passport

  1. Confirmed Online Appointment for Passport Renewal
  2. Duly Accomplished Application Form
  3. Original old passport
  4. Photocopy of Passport Data page
  5. Affidavit of Explanation
  6. 7. Additional supporting documents that may be required as necessary, (i.e., PSA birth certificate, PSA marriage contract).
  7. * All civil registration documents (i.e., birth certificate, marriage contract, death certificate) must be issued by the PH Statistical Authority (PSA), while documents from the PH court must be apostilled by the Department of Foreign Affairs (DFA).

Passport Renewal

  1. Confirmed Online Appointment
  2. Personal appearance of applicant
  3. Original old passport
  4. Printout online E-passport application form with barcode
  5. Photocopy of Passport Data page 
  6. Additional supporting documents that may be required as necessary, especially for applicants with incomplete place of birth in old passport, with dual citizenship and with foreign-sounding family names to prove citizenship.
  7. All civil registration documents (i.e., birth certificate, marriage contract, death certificate) must be issued by the PH Statistical Authority (PSA), while documents from the PH court must be apostilled by the Department of Foreign Affairs (DFA).
