• Form No.2 - Renewal Regular Passport Application (Adult)

1. Confirmed Online Appointment
2. Personal appearance of applicant
3. Bring original brown passport and photocopy of passport pages 1,2,3,4, and last page
4. Printout online application form with barcode
5. Photocopy of Passport Data page 
6. PSA-issued Birth certificate to determine complete middle name. Transcribed birth certificate from the Local Civil Registrar (LCR) is required when entries in PSA birth certificate are blurred or unreadable.
7. PSA-issued Report of Birth if born abroad
8. Identification Cards
9. Additional supporting documents that may be required as necessary.
