• Form No.3 - Regular Passport Application (Minor)

1. Confirmed Online Appointment
2. Personal appearance of minor
3. Personal appearance of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child)
4. Photocopy of valid passport of either parent (if minor is legitimate child) / and of mother (if minor is an illegitimate child)
5. Printout online E-passport application form with barcode
6. Current passport of minor and photocopy of data page
7. Original and copy of the PSA-issued Birth Certificate of minor. Transcribed Birth Certificate from the LCR is required when entries in the PSA Birth Certificate are blurred or unreadable. Report of Birth duly authenticated by PSA is required if minor is born abroad
8. PSA-issued Marriage Certificate of minor’s parents.
9. Additional supporting documents that may be required as necessary.
