Applicants who availed of Dual Citizenship or Election of Philippine Citizenship under RA 9225:
  1. Identification Certificate
  2. Oath of Allegiance
  3. Order of Approval
  4. Report of Birth (for those born abroad)
  5. Additional supporting documents that may be required as necessary.

IMPORTANT REMINDERS  

Pursuant to Department Order No. 37-03, all passports unclaimed by the applicants after six (6) months from the date of issuance shall be cancelled.
